H.S. diploma required. Assist with day treatment services to geriatric clients with serious mental illness. Provide transportation and conduct home visits to clients. Experience with geriatric population preferred. Eligible to be covered under our insurance to drive a company vehicle for client transportation.
TOP/SOC PROJECT (Transitional Outreach Program/System of Care)
TOP/SOC Transitional Advisor
Bachelor’s degree in a behavioral science required. Provide targeted case management and community support services to individuals participating in the TOP/SOC Project. Provide clinical support to individual youth and family, actively collaborate with other systems of care, and conduct home visits. Previous experience in a mental health setting strongly preferred.
Bachelor’s degree in a behavioral science required. Develops a plan for community awareness and support of employment services/activities for individuals with Serious Mental Illness. Distributes information and communicates with potential referral sources. Accepts/coordinates referrals to the SE program and screens participants. Obtains interests, desires, and goals of individual regarding his/her desire to work. Identifies integrated, competitive jobs that meet goals of the individual. Provides support services to increase independence and community integration for individuals enrolled in the SE program. Previous experience with Vocational Rehabilitation and/or Supported Employment setting preferred. Ability to obtain CESP certification.
